The Sage User Enrollment/Modification process noted below is exclusively for SAPC contracted treatment providers to onboard, modify and offboard all staff that required Sage access or
practitioners that need to be billed in Sage.
The Sage User Onboarding/Offboarding and Privilege Management guide provides the workflows for the steps noted below. The Sage User Role Descriptions guide provides details as to the
types of access that may be requested.
To obtain access to Sage, there are four (4) unique processes.

C Number Creation
- Submit a request for a C Number at https://apps.dhs.lacounty.gov/cef.
- If the user previously had a C Number, do not attempt to request a new one, email sageforms@ph.lacounty.gov for assistance with reactivating or reassigning the C Number.
- Once the C Number is approved/reactivated, SAPC will email additional instructions to the agency Sage liaison.

Sage Training
Providers can access all Sage training sets including ASAM Criteria training by registering for a SAPC-LNC account (c number not required to create an account). Sage training
sets consist of the required learning modules needed for Sage access.
How to register for an account:
- Go to https://www.sapc-lnc.org/.
- Click Register (top right-hand corner).
- Select SAPC Provider.
- Follow prompts to complete registration.
- Once registration is complete, click the URL of the training set.
- At the bottom of the trainings set page, click ENROLL NOW button to enroll in a training set.

Users should complete the relevant Sage training set based on the user role

After successfully completing the Sage training set, the user will be required to save a PDF certificate of completion, which will be attached to the case by the Sage Liaison.
This is required as confirmation of completed training and must be uploaded to the Sage User Creation form.

Sage Registration
- All user creation, modification, and termination requests will be submitted directly through the Sage Help Desk
online portal by the agency Sage liaison using the:
- "Request Something" option then
- Selecting "SAPC Sage User Creation Form" (which is similar to the "Modify a Medical Record" request).
- Only the Sage Liaison(s) for each agency will have access to submit the requests.
SAPC will email each liaison a User Creation Job Aid once we receive the C Number request, which should be reviewed to avoid common errors on the User Creation form.

Any errors on the Sage User Creation Form or missing required supporting documents will delay the process and may require the form to be rejected and the liaison
to create a new request and/or new credentialing verification and will create unnecessary delays in creating a Sage account.

All license and credentialing information must be attached to the Help Desk case for SAPC to process the request.

Please ensure ALL required training (training set and ASAM criteria) are completed beforehand.
