San Gabriel Valley residents can
find public health services at our two health
centers: Monrovia Health Center and Pomona
Center for Public Health.
Service Planning Area 3, or SPA
3, serves the communities of Alhambra, Altadena,
Arcadia, Azusa, Baldwin Park, Claremont, Covina,
Diamond Bar, Duarte, El Monte, Glendora,
Irwindale, Monrovia, Monterey Park, Pasadena,
Pomona, San Dimas, San Gabriel, San Marino,
Temple City, Walnut, West Covina, and others. |

Important Updates:
The Pomona
Center for Public Health building is
currently under construction, effective
07/11/2022. DPH is currently providing
limited clinic services, but the Parktree
Community Health Center is closed
during construction.
Please note that the 7th street entrance is
closed. Enter on 8th street
to access services.
Nurse Walk-In clinic services at
Monrovia Health Center are
closed effective 6/1/2020. Tuberculosis (TB)
services and some vaccination services remain open.
Please note that the phone lines may be
busy at times due to a high volume of calls for
COVID-testing appointments.

The Pomona Wellness Community offers a place for
community members to gather, find resources, or
participate in wellness activities. At the
Wellness Community, there are a variety of
activities such as physical activity classes,
nutrition and health resources, and other
support resources. All activities are FREE and
open to the community. |

Greater San Gabriel Valley (Health Consortium).
The mission of the Health Consortium is to
strengthen the health care safety net and
optimize seamless access to high quality
physical health, mental health and substance use
disorder (SUD) services in the Greater San
Gabriel Valley (SGV). |

This report presents a snapshot of the health of the population of
Los Angeles County overall and in each of the county's eight SPAs.
Health indicators in this report include health risk behaviors,
limited access to services, and health damaging social, economic,
and environmental conditions. |

LA County Mortality Report |
In this report, data are presented on the leading causes of death
and premature death in Los Angeles County. Data on mortality trends
are presented for the period of 2008-2017, the last year for which
data on all deaths are available. |

Beginning December 15, 2013, there will be a fee for all immunizations: $15 per child or adult or $30 for a family with 2 or more children. If you have health insurance or a regular health care provider who covers immunizations, please contact your regular health care provider for the immunizations. If you cannot pay this fee, please let the clerk know. No child will be denied vaccines due to inability to pay.
