STDs diagnosed in patients who reside in LA County should be reported to the LA County Department of Public Health STD Program using the
STD CMR form,
The new form can be filled out on a computer and then printed, or printed and filled out by hand (see sample ) . After completing the CMR form, you may return it to the STD Program by either:

California law requires health care providers to report chlamydia (including LGV), gonorrhea, PID, and chancroid within 7 calendar days of diagnosis and to report syphilis within 1 working day. The reporting of STDs does not require patient consent and does not contradict the Health Insurance Portability and Accountability Act (HIPAA) Privacy Rule.
