The COS model in California AOD systems is based on the concept that AOD dependence is a chronic illness, which has been defined by the Improving Chronic Illness Care Program as “any condition that requires ongoing adjustments by the affected person and interactions with the health care system.” Consistent with that definition AOD dependence should be addressed in a manner similar to other chronic illnesses such as depression, hepatitis C, HIV/AIDS and asthma.

In 2004, the Institute for Research, Education, and Training (IRETA) facilitated a leadership group to examine the assertion that substance abuse is a chronic illness. They established principles of care for development of new systems to treat addiction, one of which is the following:
