A comprehensive discussion of the duties and responsibilities of the Case Coordinator, demonstrating how Case Coordination is an essential part of the patient care plan development process. Participants will explore necessary steps for making successful referrals and developing a community resource list. It is strongly recommended that participants be currently assigned as Case Coordinators.
