Medical Marijuana Identification Program Appointment Information
To schedule an appointment, call 1-866-621-2204 (within LA County only).


Appointments are STRONGLY recommended.  Walk-ins are only accepted as time allows and wait times may be very long.  No appointments or walk-ins will be processed after 3:30 pm.

Before Scheduling an Appointment You Must Obtain the Following:
  1. A written recommendation from your physician recommending the use of medical marijuana. Ensure a copy is in your medical file.
  2. Call to schedule your appointment at 1-866-621-2204.
  3. The physician's license will be verified with the Medical Board of California or the Osteopathic Medical Board of California.
At the Time of Your Application Submission, You Must Present the Following:
  1. The original written recommendation with an original ink signature from your physician on official letterhead, prescription pad, or Written Documentation of Patient's Medical Record Form, including Physician's California medical license number.
  2. A completed original Medical Marijuana Program Application/Renewal Form 9042.
  3. A government-issued photo identification (i.e. California State Driver's License, a California State ID Card, United States Passport, Veteran's Administration ID Card).
  4. Qualified patients must reside in Los Angeles County and must provide proof of residence. This proof must have patient's name bearing current physical address within Los Angeles County and must consist of one of the following documents:
      -A valid California motor vehicle registration.
      -Billing Statement: current utility bill, bank statement, cellular telephone bill (not older than two months), or a current rent/mortgage agreement.
      -Insurance Document: vehicle, property, life or health insurance.
      P.O.Box address is not acceptable for address verification.
  5. Non-refundable application fee of $153.00 (or $76.50 for approved Medi-Cal participants and must bring Medi-Cal Card). Acceptable forms of payment are cash, check or money order and payable to "Los Angeles County Dept. of Public Health". If you choose to pay by cash, exact change is appreciated.
    Application fees are non-refundable.
  6. Caregivers must accompany the patient when submitting the application. The fee for a card for your caretaker is the same as the patient fee.
After Submitting Your Application
  1. The physician's signature listed on the Original Letter of Recommendation will be verified with the physician's office and will be the only signature accepted for authorization.
  2. You can choose to have your Medical Marijuana Identification Card (MMIC) sent by Certified Mail or schedule an appointment for a card pick-up.  It can take up to 35 days to receive your MMIC. If the MMIC gets lost in the mail, you will need to re-apply following the same procedures listed above.
